Behavioral Health Screening
Trauma-informed mental health assessment using validated tools (PHQ-9, GAD-7, PSC-17) with immediate stabilization support when needed.
Care Coordination
Seamless referral management and follow-up coordination with primary care providers, specialists, and mental health services to ensure continuity of care.
Family Support & Peer Navigation
Dedicated peer navigator support for foster families, including appointment coordination, transportation assistance, and connection to community resources.

What "Enhanced Integrated Care" Means for Families
Traditional foster care health services are fragmented — a medical visit here, a behavioral health referral there, with little coordination in between. AnchorCare's EICM brings all of these together. Our multidisciplinary team conducts a unified assessment, builds a shared care plan, and actively coordinates follow-through across every system involved in a child's life.
